Adult Day Programs

Helping people to live independently in their community for as long as possible is key to promoting wellness of the mind, body and spirit. That’s why Loretto offers Adult Day Programs that supplement the services individuals are already receiving and assist them with medical conditions they’re unable to manage by themselves.

PACE CNY:

PACE CNY provides a wide spectrum of care and services in a potential participant’s home, day health centers, hospitals or long-term facilities. Individuals may receive any number or combination of services as they fit into their personal plan of care developed by the team with the participant. Services are provided or arranged by PACE CNY, a program of the Loretto Family of Care. PACE CNY helps frail seniors remain in the community with support services. Some services include:

Recreation services
Social services
Nutritional services
Transportation

Daybreak, Adult Medical Day Program:

Our Daybreak Program is for adults 21+ years old in need of assistance with a medical condition but still living in the community. Some key areas of focus are on preventing and minimizing hospitalizations, including:
