"I was definitely a little down and my health was failing. I was afraid I’d end up in a nursing home. But, thanks to PACE CNY, I can live in this community and still get the care I need."
We place great value on the family unit because within it, each resident’s story has been built throughout his or her life. This is where it can be best understood. We know the resident’s situation impacts the whole family.
Therefore, our care focuses on providing support, assistance, and relief to all family members when adjusting to the effects of social, emotional, and physical losses of our loved ones.
- Initial and annual care plan review meetings are held with the resident, family, and interdisciplinary team.
- Resident or family can request a meeting with the team at any time. (Arranged and facilitated by the Social Worker.)
- Ability to participate in Family Council, a forum to discuss overall facility policies and procedures and offer suggestions to improve quality of life in a constructive manner. (Held monthly and facilitated by the Ombudsmen.)
