Coordinates and provides medical services for the participants enrolled in the Adult Medical Daybreak Program.  Improve and support nursing practice and direct participant care through roles such as educator, clinical expert, consultant, and researcher to assure quality medical services

  • Current CPR certification
  • Must possess an RN’s license 
  • Must be able to supervise LPN and CNA staff
  • Experience with the elderly and disabled strongly encouraged
  • Must be able to interact with community professionals such as physicians, home care agencies, case managers etc.
  • Previous experience as a nursing supervisor strongly encouraged
  • May be required to walk, sit, lift, reach and move equipment and assist participants
  • Must be able to speak, understand, read and write the English language
  • Must be comfortable and appropriate/respectful toward the elderly, the medically complex and people with disabilities who may have challenging behaviors
  • Must be able to manage multiple tasks and work in a busy and sometimes noisy environment while maintaining composure and observing the rights of the participants
  • Strong written and verbal communication skills
  • Ability to teach/instruct and counsel participants with physical, sensory and mental impairments
  • Ability and knowledge to respond in emergency situations
  • Provide health assessments of participants annually, semi annually, in emergencies and as needed.
  • Monitor overall health of participants to assist them in maintaining or improving their health.
  • Supervise LPN and CNA staff (and ancillary staff as necessary) to insure DOH regulations are met and quality care is provided.
  • Develop and review medical plan of care for each individual participant upon admission, every 6 months, annually and if there is a significant change.
  • Communicate with Physicians, and other health care providers regarding participant medical issues as necessary.
