Overview: 

Provide leadership for aide activities during shift. In cooperation with other team members, assure that residents/clients are provided therapeutic and rehabilitative measures to maximize independence.  Monitor and manage staff to provide assistance to clients/residents with personal hygiene. Monitor adherence to all policy and procedure to assure compliance with DOH and all other pertinent regulations.

Qualifications: 

Current New York State license in good standing as Practical Nurse or equivalent experience. 2-4 years recent experience in provision of services to acute/long term frail elderly. 2-3 years supervisory experience in related field, preferably a union environment. Familiarity with medical transcriptions . Flexible schedule. Knowledge of long term care regulations.

Responsibilities: 

Upon new admission/re-admission, meet with residents and set up personal care schedules and needs. Assure effective communications between direct care staff and client/resident families. Collaborate with Clinical team regarding progress on hospitalized residents. Prepare for discharges.  Assist with Family meetings. Monitor all medication documentation to assure accuracy, timeliness and completeness. Address deficiencies appropriately.  Manage physician's medication change orders. Assure follow-up documentation is complete. Obtain all required signatures.  Monitor and manage direct care staff effectiveness. Maintains staff development plans addressing deficiencies, including hiring, disciplines and terminations as required. Maintains confidentiality.  Conduct periodic staff meetings to provide information regarding policy and procedure, resident/client care issues, and other facility/program related communications.
