Overview: 

The Manager, Home Care RN is responsible for ensuring the provision of high quality home care services. Supervises the home care team and its practices to assure safe, effective, and efficient home care is provided by the home care team of RN's, LPN's, physical therapists, occupational therapists, special therapists, respiratory therapists, social workers, dietitians, home health aides, and personal care aides. Implements and provides guidance on agency policies and procedures. Identifies areas of improvement or concerns and brings them to the attention of the Director of Clinical Services. Oversees the orientation, supervision, direction, and evaluation of nursing, therapy other professional and paraprofessional staff.

Qualifications: 
  • 5+ years’ experience in home care.
  • 2+ years’ supervisory experience.
  • BSN preferred or working on BSN with a minimum of 2 years of community health nursing experience.
  • Licensed and registered as a professional nurse in good standing in NYS.
  • Graduate of an approved Professional Registered School of Nursing.
Responsibilities: 
  • Supervises an interdisciplinary team of home care RNs, LPNs, Physical Therapists, Occupational Therapists, Speech Therapists, Respiratory Therapists, Nutritional Therapists, Social Workers, home health aides, and personal care aides.
  • Completes an in-home and a written performance evaluation of staff supervised: at the end of the three-month probationary period; and once per year on or within four weeks of the staff member’s anniversary date. The in-home evaluation includes an assessment and observed demonstration of the delivery of care.
  • Reviews staff’s previously set goals and establishes future goals for the upcoming year, during the annual performance evaluation of each staff member.
  • Monitors staff’s completion of mandatory annual in-services (PACE Reorientation, Loretto Reorientation, Fire Safety), annual health evaluations, and the completion of the monthly safety in-services; documents status and discusses with staff during the annual performance evaluation.
  • Facilitates or performs education, training, and competency evaluations for appropriate staff for special care needs (i.e. Foley care, leg bag care, lift equipment).
  • Works closely with scheduling team regarding all aspects of documentation, scheduling, and information management, and meets with scheduling team as needed. Addresses all scheduling inconsistencies (i.e. missed aide visits) and staff documentation issues.
  • Works with scheduling to ensure appropriate staffing for complete 24/7 staffing.
  • Performs ongoing review of patient cases and delegation of staff assignments to other staff (i.e. RNs, LPNs, CHHAs) to ensure appropriate utilization of staff.
  • Investigates and evaluates problems and situations that arise including incidents, grievances, and events involving level II reporting.
  • Meets with neighborhood staff monthly, or as needed, to be kept informed of patient status, staff issues, and scheduling concerns and records meeting minutes which are kept for one year.
  • Meets with professional staff daily (business days) to be kept informed of patient status, staff issues, and scheduling concerns and records discussion on morning report notes; keeps the discussion notes for one year.
  • Effectively communicates and collaborates with the IDT on a daily basis (business days), and more often, as needed, as evidenced by the morning meeting minutes.
  • Recognizes contributions and celebrates accomplishments of staff.
  • Interprets, updates and communicates information on policies, procedures and regulations.
  • Investigates, follows up, and resolves customer service concerns, in accordance with PACE CNY Grievance Policy. Provides documentation to the Director of Clinical Services.
  • Demonstrates effective communication skills while working cooperatively with Home Care staff, IDT and outside providers.
  • Demonstrates initiative and compliance by attending competency fairs two times a year and mandatory in-servicing.
  • Promotes team building by demonstrating an active role in staff meetings, orientation of staff and welcoming new staff.
  • Promotes professional growth, the profession of nursing and the program, when possible, by participation in profession associations, educational conferences, volunteer activities, etc.
  • Demonstrates adherence to all compliance policies and procedures and the code of conduct. Promotes, fosters and displays compliance oriented behavior in the workplace.   Adheres to the mission and philosophy of PACE CNY and the Loretto Corporation.
  • Monitors the appropriateness of home care services in place to meet participant needs.
  • Works collaboratively with other team members to facilitate meeting participant care needs.
  • Performs medical record review to assess the documentation compliance in relation to agency and DOH regulations.
  • Performs a rotating on-call duty, accepting calls from the answering service, triage RN service, Visit RN, scheduling, and staff during non-business hours. Performs occasional nursing visits during non-business hours to support participants at end-of-life.
  • Ensures efficient utilization of staff by monitoring staff-participant ratios and participant acuity.
  • Works to minimize staff overtime and mileage by monitoring schedules and assignments.
  • Analyzes customer satisfaction survey results to develop performance improvement activities.
  • Identifies situations in which home health aide attendance would be beneficial at care planning/reassessment/family meetings and coordinates home health aide attendance.
  • Works with the Director, Patient Services, to prepare, implement, and follow up on DOH survey deficiency report findings.
  • Works closely with the Director, Patient Services on performance improvement and regulatory compliance monitoring of agency activities.
  • Analyzes neighborhood staff’s documentation for regulatory compliance and quality. Follows up with staff as issues arise.
  • Develops and implements policies and procedures to improve organizational function.
