Overview: 

The RN Home Care Manager is responsible for ensuring the provision of high quality home care services. Supervises the home care team and its practices to assure safe, effective, and efficient home care is provided by the home care team of RN's, LPN's, physical therapists, occupational therapists, special therapists, respiratory therapists, social workers, dietitians, home health aides, and personal care aides. Implements and provides guidance on agency policies and procedures. Identifies areas of improvement or concerns and brings them to the attention of the Director of Clinical Services. Oversees the orientation, supervision, direction, and evaluation of nursing, therapy other professional and paraprofessional staff.

Qualifications: 
  • BS in Nursing or other health related field.
  • 2 years’ experience in Home Care preferred.
  • Current NYS RN license, current driver's license and vehicle, excellent communication skills including conflict resolution and team building skills.
  • Experience managing CHHA's preferred.
Responsibilities: 
  • Supervises an interdisciplinary team of home care RNs, LPNs, Physical Therapists, Occupational Therapists, Speech Therapists, Respiratory Therapists, Nutritional Therapists, Social Workers, home health aides, and personal care aides.
  • Completes an in-home and a written performance evaluation of staff supervised: at the end of the three-month probationary period; and once per year on or within four weeks of the staff member’s anniversary date. The in-home evaluation includes an assessment and observed demonstration of the delivery of care.
  • Works closely with scheduling team regarding all aspects of documentation, scheduling, and information management, and meets with scheduling team as needed. Addresses all scheduling inconsistencies (i.e. missed aide visits) and staff documentation issues.
  • Performs ongoing review of patient cases and delegation of staff assignments to other staff (i.e. RNs, LPNs, CHHAs) to ensure appropriate utilization of staff.
  • Investigates and evaluates problems and situations that arise including incidents, grievances, and events involving level II reporting.
  • Meets with professional staff daily (business days) to be kept informed of patient status, staff issues, and scheduling concerns and records discussion on morning report notes; keeps the discussion notes for one year.
  • Recognizes contributions and celebrates accomplishments of staff.
  • Investigates, follows up, and resolves customer service concerns, in accordance with PACE CNY Grievance Policy. Provides documentation to the Director of Clinical Services.
  • Promotes team building by demonstrating an active role in staff meetings, orientation of staff and welcoming new staff.
  • Promotes professional growth, the profession of nursing and the program, when possible, by participation in profession associations, educational conferences, volunteer activities, etc.
  • Demonstrates adherence to all compliance policies and procedures and the code of conduct. Promotes, fosters and displays compliance oriented behavior in the workplace.   Adheres to the mission and philosophy of PACE CNY and the Loretto Corporation.
  • Works collaboratively with other team members to facilitate meeting participant care needs.
  • Performs medical record review to assess the documentation compliance in relation to agency and DOH regulations.
  • Performs a rotating on-call duty, accepting calls from the answering service, triage RN service, Visit RN, scheduling, and staff during non-business hours. Performs occasional nursing visits during non-business hours to support participants at end-of-life.
  • Analyzes customer satisfaction survey results to develop performance improvement activities.
  • Works with the Director of Patient Services, to prepare, implement, and follow up on DOH survey deficiency report findings.
  • Develops and implements policies and procedures to improve organizational function.
