Overview: 

Under guidance and direction of Nurse Manager, participate in planning, organizing and reviewing

operations. Provide direct skilled nursing care to residents. Serve as primary support to Physician, Nurse

Practitioner/Physician Assistant, Nurse Manager or Clinical Coordinator. Adhere to all Corporate

Policy/Procedures and third party agency regulations, following accepted nursing practice standards.

 

Qualifications: 

- Graduate of accredited school of nursing, college or university.

- Licensed and currently registered in NYS as a registered professional nurse.

- (1-2) years experience in long term/acute care, prefferably in a geriatric setting.

 

Responsibilities: 

Compliance

-Assure adherence to bargaining unit agreement by staff. Notify supervisor of issues in timely,

professional manner.

-Comply with annual orientation/re-orientation, health assessment, competencies, etc as per

policy/procedure and regulatory requirements.

-Perform resident care according to policy/procedures/standards and adhere to all third party

agency regulations.

 

Corporate Participation

-Actively support Corporate goals and initiatives through personal goal setting and monitoring

progress.

 

Documentation

-Assure care plan is updated accurately, completely and timely as required by change in status,

physician orders, regulations, etc. Communicate updates effectively to interdisciplinary team..

-Assure MARs - are complete, accurate and timely.

-Collect and document data relevant to resident's current health status, medical history,

discharge plan needs.

-Document resident care information in accordance with Health Information procedures,

adhering to regulatory requirements.

 

Employee Development

-Actively participate in orientation for new employees as requested.

Assist with in-services/education of staff. Comply with Corporate/Regulatory in-service

requirements.

-Establish staff priorities for nursing actions according to resident needs.

-Provide direction to staff to meet resident care needs.

 

Equipment/Loretto

-Assure medication carts are stocked and maintained prior to and upon completion of medication pass

-Utilize Loretto equipment as intended to acheive maximum useful life. Notify maintenance of issues in a timely manner.

 

Quality Assurance

-Assure nursing care actions are completed by staff accurately, completely and in a timely

manner.

-Effectively support Quality Assurance activities though data gathering, auditing, analysis when

requested to do so.

-Monitor resident/family compliance to teaching plan. Initiate corrective action as required.

 

Resident/Client Care

-Actively participate in development of realistic, measureable, time bound goals for the resident

and staff.

-Assist residents with ADLs and special care needs as indicated by Nurse Manager, Clinical

Coordinator, Interdisciplinary team, etc.

-Assure appropriate inventory level of nursing supplies to meet needs of residents. Assure

rotation of stock as required.

-Monitor and document resident response to medications. Intitate appropriate action based on

observations.

-Obtain, transcribe and assure Physician's o - rders are implemented accurately.

-Perform rounds to assess/evaluate resident care. Report changes and document appropriately.

Promote an environment for the residents that acknowledges resident's rights, individuality and

encourages maximum level of independence.

-Provide instruction and guidance to family, residents, and care givers on medications and plan

of treatment.

-Provide/ assist with provision of direct care needs for residents according to care plan/Physician

orders/ resident needs.
