Helping people to live independently in their community for as long as possible is key to promoting wellness of the mind, body and spirit. That’s why Loretto offers our Daybreak Adult Medical Day Program to supplement the services individuals are already receiving and assist them with medical conditions they’re unable to manage by themselves. Our Daybreak Adult Medical Day Program is for adults 21+ years old in need of assistance with a medical condition but still living independently.

 We focus on prevention to minimize hospitalizations, including:

  • Medication management, insulin shots, etc. through our nursing services

  • Nutritional services such as our daily lunch

  • Physical, occupational, and speech therapy

  • Personal care such as assistance with toileting

Anyone can contact Daybreak to inquire about enrolling. Eligibility will be determined after an assessment by a registered nurse. Once a physician’s orders are obtained, Daybreak will set up a regular attendance schedule for the individual. Financing options include Medicaid, participating insurance companies and private pay. Our health care team consists of:

  • Registered Nurses/Licensed Practical Nurses

  • Registered Dietitian

  • Certified Nurse’s Aides

  • Therapeutic Recreation Specialist

  • Physical, Occupational and Speech Therapists

  • Case Managers

Our admissions process includes:

  • Phone interview to determine type of service needed

  • An assessment at our program for our RN to determine needed services and eligibility

  • Obtaining orders from the resident’s PCP to complete enrollment

  • Continued eligibility by attending a minimum of 1 day/week
